Athletic Files and Forms

Every year students must complete the athletic packet.  It will remain in effect until the end of the school year.  Additionally for each sport, students need to complete the Warning and Safety Guidelines for that specific sport.  All forms are available online by clicking the "athletic packet" link and used for both Middle School and High School athletics.  If you have questions, please call 509-687-9585.
